Required Paperwork

After completing your online registration, you will need to submit additional required paperwork for each camper attending IdRaHaJe, described below. Please submit this paperwork at least one week prior to check-in by uploading it to your camper’s registration on our online registration portal. We can also accept submissions via email or mail.

Please save a copy of all forms and bring them to check-in, even if previously submitted.

Current IdRaHaJe Physical Form

• • Mandatory for ALL IdRaHaJe Summer Campers
  • Must be dated within the previous 24 months at the time of the camp session
  • Must be completed and signed by a health care professional
  • Must be on IdRaHaJe’s specific Physical Form

Immunizations

• When a registration is submitted, the IdRaHaJe Registration team will access Colorado’s Immunization System to check a camper’s immunization status.
• All of Colorado’s required vaccines for camp attendance must be included.
• If you do not immunize or opt out of some of the required immunizations for non-medical purposes, your health care provider will need to sign the Immunization Exemption Form OR you will need to complete the Online Education Module at this link.
  • The exemption form or certificate of completion for the module will need to be uploaded to our registration system.
  • These must be completed and submitted annually.
• If you do not immunize for medical purposes, please follow the directions provided HERE.
• For campers who do not have immunizations on file with Colorado’s system, parents can provide proof of immunization by uploading documentation from a doctor or another state.

IdRaHaJe Medication Form

• • Mandatory for Campers bringing any type of medication (including prescription, OTC medications, vitamins, homeopathic remedies, essential oils, medical ointments, etc.)
  • Must be dated within the previous 12 months at the time of the camp session
  • Must be completed and signed by a health care professional

Whitewater Rafting Waiver (For NARROW WAY and LODGE CAMPERS only)

• • Required for all Lodge Campers and all Narrow Way Campers (except Backpacking Adventure) to participate in Whitewater Rafting.
